The easiest way to do so is using one of my favorite plugins, Gravity Forms.
Gravity Forms is the most advanced WordPress forms plugin, allowing you to create contact forms, lead capture forms, and, of course, order forms. Unlike many other forms plugins, it has an easy to use, visual drag and drop interface. And includes many advanced fields and options, such as data selection, multi-page forms, and conditional logic.

Creating WordPress Order Forms using Gravity Forms
Here are written instructions you can use to follow along:
1. Add the appropriate Add-On
Go to the “Forms” drop down in the left hand menu of your WordPress backend and select “Add-Ons”.
Find the payment processing add-on you’d like to use and add it.
For most of you, that will be PayPal.
2. Configure Payment Processor with Gravity Forms
Go back to the Forms drop down menu and select “Settings”. Choose “PayPal” (or the appropriate payment processor you’ve added) and follow the instructions for integrating with Gravity Forms.
3. Create a New Form
Click on “New Form” from the “Forms” drop down.
You can add any fields you’d like. Primarily, you want to add the pricing fields. Below is an explanation of each pricing field option:
- Product – allow users to select a product to order in the order form.
- Quantity – add a quantity field to allow users to select how many of each product to purchase.
- Option – allow users to select additional options for their order. You can set a different price for each option.
- Shipping – set shipping rates.
- Total – allow users to see the total amount of their order before placing order.
4. Create PayPal Feed
The final step is to turn the form into a PayPal form.
On the left hand menu under “Forms”, select PayPal. Click the link to create a new PayPal feed.
You’ll then select the form you created. Then, you’ll map your form fields to the appropriate PayPal fields. For more advanced users, you can edit some PayPal settings, such as what URL to take users to when they cancel an order.
Once you’ve completed this step, your order form is ready to be added to a page.
